Hi!

I'm gathering all my supporting documents and evidences to upload online for my PMV to Partner visa. I know Health and Character assessment aren't necessary cos already done both for my PMV, but do I need to fill up Form 80 again? And Form 1221 is a new one on the checklist. I would like to know if it is required as well. Many thanks.

We never supplied the form 80 again since we supplied it at PMV stage. Never supplied the form 1221 either.
