Some Visa Application related questions

I have some questions regarding PR Visa. Can anyone of you kindly clarify.
1) While attaching documents online after visa payment,
a) Is it mandatory to take photocopies of originals and get them certified/ attested for upload
OR
b) Just scanning the original document (in colour) directly and uploading is sufficient (without getting certified/attested). Is this agreed by DIAC & mentioned somewhere in its website ? Just wanted to be sure that choosing this option is perfectly valid & does not delay my visa process.
2) I have applied for 190 visa (NSW) including my spouse & child in my application.
a) once the visa is issued, what is the accepted timeframe before which we have to be in Australia at least once.
b) In that case, within the mentioned period
Is it mandatory that all the 3 applicants need to visit Australia.
OR
Anyone of the applicant being there is enough.
OR
Not anyone, but at least the Primary applicant MUST visit within that time.
3) I heard from my friend (who enquired with an agent) that, one have to show sufficient bank balance for a 190 visa, which is not needed in the case of 189 Visa!
Is it True? If true, how much one has to show and at which point during the Visa process will DIBP ask for it. I have submitted my online 190 Visa application yesterday and it never requested for any such details & neither does the "190 visa Documents checklist" mention about it. Kindly clarify
4) Once the DIBP decides to offer Visa, will they call at the nearest Australian Embassy for an interview & identification ? What is the process ?
