Hi
I have never heard of this form, who told you to get it.
As she is your girlfriend you are unable to sponsor her, however you can provide a letter inviting her to visit, in which you can state that you are able to offer financial support, board and lodging.
The application can be made on line, so it is relatively easy to apply, but you still have to provide compelling evidence for her to return to the Philippines after her trip.
Such evidence can be ;
A letter from her employer stating that she has a job to return to
Bank book showing access to substantial funds
Property titles in her name
Family commitments.
Hope this helps

I have done a couple of visitor visas for Fiipina's one for my fiancee a while ago and recently one for my fiancee's sister.

Both were students at the time.

I supplied:

Stat dec stating I would provide financial support, accomodation and fund her return tickets.
My payslip/bank statements/house rates notice.
Invitation letter
Her student ID
Her class schedule

Both were granted.
