Applied offshore, approved onshore?

Hi guys can anyone tell me if It's possible for a offshore applied visa to be granted on shore? If not (which is my assumption) how long after you were told to leave Australia was your visa granted. For eg; you return home sent your passport for stamping and revived it when?
