Attaching additional info to FORM 888?

The form 888 I downloaded (from the official document checklist page) doesn't have any space to put additional information (no lined pages like the application forms). Can I just attach a normal sheet of paper? Does this need to be signed separately?

i just attached ours in a normal document and converted it into one pdf
