Query regarding GST exemption declaration

I have applied for general skill assessment today. In the required documents to upload list, there is an item titled somewhat like GST Exception Declaration. My question is, will it suffice to make the declaration in a white paper and just signing it or it has some other legal requirements like stamp paper and notarization?
