Form 80 for a 189 visa?

Is Form 80 required for a 189 skilled visa? If so, am I the only one who needs one, or will I have to fill one out for each of my children, as well? From my understanding, it is a pretty intensive form, so I figured I should get to work on it now, before I am ready to lodge my 189 application.

They asked for all adults under my application to fill in one, not needed for those under 18.
Bear in mind, I remember reading somewhere that not all applicants are asked to fill in this form.

Thank you for the reply. Yes, it would just be me filling it out (I took a look at the form...it's so long!). And that's what I thought, that it was only required if they ask for it. I'm so close to being ready to lodge the application, that I don't want to take any chances. I'm from the United States, and am a secondary school teacher. So, I don't know if they will ask or not.

Has anyone else had them ask for it when filing a 189? Or have not had them ask? I'm curious to find out when they would ask for it (or why they do).

It's purely up to you whether you want to fill it in now and upload it with your initial application, or whether you want to wait until they start assessing your application and see if they ask for it.
I was given 4 weeks/1 month to fill in and upload.

I just lodged my application and I think it is better to fill the form now (why waiting for others to ask you to do this?)
