How did you present your De-Facto application?

Hey guys,

I am in the final stages of getting my De- Facto application completed before sending it off.
The one thing I am worried about is how to present it?
Could you let me know how you presented it?
Did you put it in a folder? Plastic slips? All advice will be awesome!
