Visitor Visa - Error Message

I started a Visitor Visa Online application for my Dad (who is in the Philippines) today and when I got to page 4/20 I received an error message preventing me to proceed with the application. Refer to attached screenshot.

I have applied for 2 Visitor Visas on his behalf in the past and have been approved both times. He has never overstayed, nor was he refused or got his visa cancelled. He abide by the conditions of his visa so I am surprised to get this error message.

I called immigration but they wouldn't give me any details unless they speak with my Dad directly. The lady suggested that if I believe this to be an error I should lodge a technical support form which I did.

But just wondering what could've happened here and if any encountered the same and what they have done etc.

Thanks in advance for your feedback.

Has his last visa actually expired, or is it still usable? It may be multiple entry?

Hi JandE, yes it has expired. Last application and entry in 2017 and visa expired in April 2018. They are only really here for 3 weeks max every time.
