Form 40 for 801

Hi everyone,

Sorry for so many questions of late, but I really appreciate the advice I receive here

Is the Form 40 a requirement for the 801 application?

I vaguely remember something of similar content that we filled out for our 820 and PMV.

Is it required again?

Thank you for any help,

Sarah.

Form 40 is for sponsorship of a parent, aged dependent relative, remaining relative or carer, I think.

However, we just did our 100 and didn't upload any forms, just our evidence and statements. You have to answer questions in the application before submitting evidence though. Maybe that's what you mean?

No this is an option for upload under the documents section.

Oh damn! It’s not form 40 it’s form 80!!!! I’m so sorry.

Is that required for the 801 application?

We didn't include form 80. I saw that it was an option though on the upload screen, however we didn't complete the form and upload it. I could be wrong, but I think that all the questions in form 80 are asked in the online application you fill out before uploading evidence.
