I am a dentist with regional sposorship and job is SA. I have Aussie registration but am 63 years old. Do I just apply for an 457 visa Temp stay or do I have any chance of getting an 857 permananent res visa. I would love to stay in Aus long term. I would be grateful for any advice.

I reckon you might be stuck with just getting a 457 visa as the age exemptions that are available for the ENS/RSMS usually only extend to people being 60 years of age.
Regional Sponsored Migration Scheme (Subclass 119/857)

However you could try and get some sort of an indication from DIAC as they may look at someone older seeing as you're going to be looking at a regional area and the government has already raised retirement age to 67 from 65 and there's always talk of wanting to have people working until older, and not so many professionals prepared to work regionally.

You might want to have a chat with a good agent and get a view and perhaps assistance in putting the exemption case forward, bearing mind that it'll likely mean putting an application in and if it is refused you stand to lose your application fee.

Also, unless you're already here using a 457 as a qualifying visa it'll be the 119 offshore visa you would need to apply for and have to be outside of Australia for it to be granted.
