Is form 80 always required for 309?

Visa application submitted a week ago, got a confirmation email from embassy saying the application has been assessed as valid application and then goes on to list documents required if not already provided. Nowhere is mentioned form 80.

I've seen this form and it looks quite daunting and would prefer not to have to do it if it's not needed. Has anyone had any recent experience in the philippines where the embassy asked for form 80?

They do not always require them. What you can do is fill it out anyway, so you can provide it quickly, if required. It's not as daunting as it looks. You can fill out most of it quite quickly, making sure that your responses are consistent with the information you have provided in the other forms. The main issues for most people are dates of previous travel, previous addresses and employment history. It's better you take your time digging up all that info and do it now rather than having to provide it in a hurry later on.
