statutory declaration from France


I received an invitation to apply for a 189 visa and am working on it at the moment (I am in France).

As I am on a "de facto" relationship", I shall do a statuatory declaration to claim it is a genuine relation, but don't understand how it works really

Does somebody knows who, outside from Australia, is "allowed" to witness this kind of declaration? A notary, A mayor?...

Should it be written on simple paper or should I use a specific form?

Should I write the declaration with my girlfriend, or should it be done by some friends or parents who'll write "I 've known Nicolas and Anne for xx years and certify they've been blablabla..."?
