name fields - urgent help please

hi experts/seniors,

pls consider following situation. if my name is:

first name: james
last name: bond

and it's written in my passport as:

first name:
last name: james bond

i am currently filling up state sponsorship application. it asks me to fill my name as per passport:

given name:
family name: james bond

it says if one doesn't have given name, they should write N/A in that field. now after completing the application, it shows my name as:

Mr James Bond, N/A

since this will have a long term impact on all my documents, i just want to clarify in which of the below two ways should i proceed:

1- as per passport
surname: James Bond
given name:
to be appeared in application as: Mr. James Bond, N/A

2 - as per actual
surname: Bond
given name: James
to be appeared in application as: Mr. Bond, James

appreciate you help in advance...

Sounds like you need to get your passport corrected if your name is not correct there. Your name in your passport needs to be correct so it matches your other official documents. AND your name on your application needs to match your passport.

Visa #1: PMV (300) through Washington, D.C
Applied: April 2013.
Visa Granted: January 2014.
Visa #2: Subclass 820 (From PMV).
Applied: End of April 2014.
Visa Granted: Early July 2014.
Visa #3: 801 (PR)
Eligibility Date: End of April 2016 (Applied a month prior).
PR Granted: Early April 2017.
