These may sound like silly questions but I'm having a bit of trouble with what some of the questions on my form 47SP mean, I've tried asking immigration but the young lady wasn't very helpful so hopefully some of you with your greater wisdom will be able to lend me a hand.

In part D question 33 it asks 'Are any of your family members included in this application for migration under 18 years of age'

I'm not migrating with anyone so first I thought I should leave it blank as it's not relevant, but after reading some things on here I've realised I shouldn't leave any question blank, so should I just tick NO?

In part H question 74 it asks 'Have you ever been granted permission to work in Australia?'
I'm on a Second year working holiday visa, so I am allowed to work, but as I have been on a first year working holiday visa and a bridging visa (all with work rights) would I have to give them as three different times I was granted permission to work, or one continuous period of time.
e.g. 1/06/2012 - 1/06/2013 and 2/06/2013 - 1/08/2013 and 2/08/2013 - 1/06/2014 OR 1/06/2012 - 1/06/2014
I'm thinking I would have to give it as 3 separate dates as they were 3 separate visas, but just want a second opinion

Lastly, part H question 75 'Have you ever been paid any benefits or received any social services from a government agency in Australia? (for example .. medicare ..)'
I've had a medicare card for the majority of my stay in Australia as the UK has a reciprocal care agreement with Australia, would I have to declare this? I haven't been paid any benefits from medicare, the one time I did visit the doctor surgery my first card had expired so they just let me pay the cheaper rate on good faith they didn't link it to my medicare number. Not sure if this question is asking whether I've actually been paid any money by them, or just whether I have access to their services.

Sorry if these seem like silly questions, a couple of things on this form have really thrown me and I'm sure other people have had trouble too so if I can help someone with some of the other questions let me know

I've never had to fill in this form, so this response is based on the documents I had to provide and general opinion:

Question 33 - "Included in the application" refers to you and anyone migrating with you. Since there's no one other than you, then you'd say NO (don't leave it blank)

Question 74 - Since there has been 3 different visas each with their own visa grant number, I'd include them all individually. DIBP will have information on all your visas to look this up anyway.

Question 75 - since the visit wasn't billed through Medicare, I'd say NO
