I need help in filling up medicare enrollment form

Hello All!

5 Days since I was granted of my temporary visa 820,and today I went to Medicare and was given a form to fill up.

First question what does "contact person's details" mean in the first section of the form?Is it me?or my husband?

Secondly,theres a section for "applicants details", is it me or my husband?

Lastly,there's a section for "bank account details",and I don't have an australian bank account,does this mean I can't enrol for medicare without it?or can I use my husbands?

It's so confusing,I'm supposed to ask the customer service there but the que was long and I really didn't want to ask much,and thought that fellows from here could help me fill up the form.

Also I forgot to ask,under the "contact persons details" question 6."Will you be listed on the medicare card as part of enrollment?If no,explain why you will not be listed (e.g.power of attorney,carer,parent arranging card for a child or a gov't authority)".

What does it exactly means?

You are the contact person and the applicant. The reason these are separate things is that sometimes you have, for example, mothers registering a new baby for medicare. In that situation, the mother is the contact person and the baby is the applicant. In this case, you are both. I'm not sure on the bank account - I had my own so didn't have an issue with that. You can PROBABLY use your husband's, but check with them first to make sure. If you can't, that just means they'll send you a check instead of direct depositing any rebate they're giving you. The answer to your last question is yes - you're going to be listed on the card. It's simply asking if you are the one enrolling in Medicare, or if you are enrolling someone else (like a child, elderly parent, etc.)
