just wondering under what circumstances would a Spouse Visa (offshore or onshore) be rejected?

Pffft, they mainly reject it if they believe the relationship is not genuine or in case of de facto visas, the couple cannot provide enough proof that they have lived together for 12 months. Some believe that if it is "almoust 12" or "only 7 but we love each other so much" is enough for DIAC, but it isn't. Check out the MRT-s website, and search for spouse visas under the decisions section. (The seach engine is a pain, but you will still find many.) They usually put down why the initial case has been rejected, what evidence has been provided and when, and explain the details.

For any visas there are eligibility requirements and so it can be if anything is not complied with, that may lead to a visa not being granted.
