we are going through this now, best to wait for them to ask you to do it - main reasons are 1. you have to prove your application is valid for the type of visa 2. then you get a case no. and/or case officer so the medicals can be sent to the right place and 3. you may get wrong tests or not enough ie. bloods, xray etc varies for some people and the type of visa and the country applying from. I know with ours we were going to to them first to save time but told by case officer and immi in australia not to do this, and just found out ours will be sent to him to do with 2 of the photos he included in the application. Best to wait, but check with the agency who will do them if you need an appointment and how long is the waiting list if they have one, if it is a long one maybe you can book in but not attend until asked to.

Though for overseas applications it is optional whether you do them to submit with your application or wait until requested, some people do them up front in the hope that it will speed processing which it could do marginally however there is also a risk attached and aside from what KandK mentions, the main issue is with time of processing.

Many visa processing times can be lengthy and so with a nedical validity of only 12 months, an applicant could end up in the situation of needing to repeat or because when a visa is granted, you are given an enter by date which is the 12 months anniversary of a medical, you may have very little time to arrange your entry.
