Documents needed for skills assessment

Hello all,
I hope to summit my documents for skills assessment in the following weeks.i am a geologist with about 4 years experience,i would like to be informed on the specific documents to summit.
what do i need to include and do to ensure i get a positive response from the skills assessment body.
Do i apply for the 176 or the 475?which is relatively easier as i qualify for both.
